Tried doing it as in procedure (including erasing the phone during transfer backup to phone, even logging in to US iCloud account), can confirm it is not working/no longer working with iOS 14.0.1 (iPhone Xs), watchOS 7 (Apple Watch Series 4). Also checked “erase target device” when restoring with imazing When I did it I found two .***.plist in the backup, one talking about age gating and the other about feature availability having been downloaded in the last 24 hours and I deleted them but idk if that did something.

After patching PLIST file, Health app says: "open the ECG app on your Apple watch to take a recording" and subtext says: "Enabled on Mitja's Apple watch" and no birth date required as before patching PLIST file.

Nop, I was needed to do both, first I unpair the watch, after I backup the iPhone, restore the backup and pair the watch, but many system apps don't work, I try to restart the watch (soft and hard reset) but the issue still there, my solution was to make a factory reset (Erase all content and settings). You have supposed to Unpair the watch before starting, that’s probably why you needed to reset it to make it work.
